As a RN Case Manager, you will be overseeing the care to residents in a caring, safe and efficient manner to meet high quality outcomes for care, Use your RN skills in managing a variety clinical situations from short term rehab to assisting residents with chronic and long term medical management. As a RN Case Manager, you will lead care planning discussions, conduct assessments, and interact with the interdisciplinary team, physicians and families.
